Referral

We accept consultations for aortic aneurysms (both descending thoracic aorta, abdominal aortic aneurysm, but ascending aortic aneurysm would be referred to cardiac surgery), vascular dissections, peripheral artery aneurysms, visceral aneurysms, carotid artery stenosis, peripheral arterial disease, dialysis access creation, wounds- diabetic, arterial or venous, thoracic outlet syndrome, venous insufficiency, arteriovenous malformations, renal artery stenosis and visceral artery stenoses.

Please provide a brief patient history and description of chief complaint pertaining to the referral.

If possible, please provide an up-to-date list of medications.
